  Background: Psychosocial stress-management interventions can reduce stress and distress and improve the quality of life for survivors of cancer. As these in-person interventions are not always offered or accessible, evidence-informed digital stress-management interventions may have the potential to improve outreach of psychosocial support for survivors of cancer. Few such digital interventions exist so far, few if any have been developed specifically for survivors of breast cancer, and few if any have attempted to explore more than 1 distinct type of intervention framework. Objective: This study aimed to develop 2 digital psychosocial stress-management interventions for survivors of breast cancer; 1 cognitive behavioral therapy-based intervention (CBI), and 1 mindfulness-based intervention (MBI). Methods: The development of the CBI and MBI interventions originated from the existing StressProffen program, a digital stress-management intervention program for survivors of cancer, based on a primarily cognitive behavioral therapeutic concept. Development processes entailed a multidisciplinary design approach and were iteratively conducted in close collaboration between key stakeholders, including experts within psychosocial oncology, cancer epidemiology, stress-management, and eHealth as well as survivors of breast cancer and health care providers. Core psychosocial oncology stress-management and cancer epidemiology experts first conducted a series of workshops to identify cognitive behavioral and mindfulness specific StressProffen content, overlapping psychoeducational content, and areas where development and incorporation of new material were needed. Following the program content adaptation and development phase, phases related to user testing of new content and technical, privacy, security, and ethical aspects and adjustments ensued. Intervention content for the distinct CBI and MBI interventions was refined in iterative user-centered design processes and adjusted to electronic format through stakeholder-centered iterations. Results: For the CBI version, the mindfulness-based content of the original StressProffen was removed, and for the MBI version, cognitive behavioral content was removed. Varying degrees of new content were created for both versions, using a similar layout as for the original StressProffen program.   Background: While adherence to cancer prevention recommendations is linked to lower risk of colorectal cancer (CRC), few have studied associations across the entire spectrum of colorectal carcinogenesis. Here, we studied the relationship of the standardized 2018 World Cancer Research Fund/American Institute for Cancer Research (WCRF/AICR) Score for cancer prevention recommendations with detection of colorectal lesions in a screening setting. As a secondary objective, we examined to what extent the recommendations were being followed in an external cohort of CRC patients. Methods: Adherence to the seven-point 2018 WCRF/AICR Score was measured in screening participants receiving a positive fecal immunochemical test and in CRC patients participating in an intervention study. Dietary intake, body fatness and physical activity were assessed using self-administered questionnaires. Multinomial logistic regression was used to estimate odds ratios (ORs) and 95% confidence intervals (CIs) for screen-detected lesions. Results: Of 1486 screening participants, 548 were free from adenomas, 524 had non-advanced adenomas, 349 had advanced lesions and 65 had CRC. Adherence to the 2018 WCRF/AICR Score was inversely associated with advanced lesions; OR 0.82 (95% CI 0.71, 0.94) per score point, but not with CRC. Of the seven individual components included in the score, alcohol, and BMI seemed to be the most influential. Of the 430 CRC patients included in the external cohort, the greatest potential for lifestyle improvement was seen for the recommendations concerning alcohol and red and processed meat, where 10% and 2% fully adhered, respectively. Conclusions: Adherence to the 2018 WCRF/AICR Score was associated with lower probability of screen-detected advanced precancerous lesions, but not CRC. Although some components of the score seemed to be more influential than others (i.e., alcohol and BMI), taking a holistic approach to cancer prevention is likely the best way to prevent the occurrence of precancerous colorectal lesions.

  Objectives Aging is associated with changes in body composition. Excess adiposity among older adults has been linked with metabolic syndromes and aggravated age-associated decline in physical functioning. Few longitudinal studies have explored the association between dual-energy X-ray absorptiometry (DXA)-derived total as well as central adiposity measures and frailty. We examined the association of DXA-derived total and central adiposity with pre-frailty/frailty among Norwegian adults after 8 years of follow-up. Design Prospective observational study. Setting Community-dwelling adults from Tromsø, Norway. Measurements Adiposity was defined by fat mass index (FMI) and visceral adipose tissue (VAT) mass assessed using DXA measures. Frailty status was assessed by low grip strength, slow walking speed, exhaustion, unintentional weight loss and low physical activity level. Pre-frail and frail participants at baseline were excluded. Sex-stratified multivariable logistic regression models were used to investigate the association. Results Participants comprised 234 women (mean age 68 years) and 146 men (mean age 69 years) attending the population-based Tromsø Study in 2007–2008 (Tromsø6) and 2015–2016 (Tromsø7). At the end of follow-up, 25.6% of the women and 27.4% of the men were pre-frail/frail. Compared with women in the lowest tertiles, those in the highest tertile of baseline FMI (odds ratio [OR] 4.42, 95% confidence interval [CI] 1.88–10.35) and VAT mass (OR 2.47, 95% CI 1.10–5.50), respectively had higher odds for pre-frailty/frailty at follow-up. Conclusion We found a higher likelihood of pre-frailty/frailty in later years among women with general and central adiposity in adulthood, highlighting the importance of preventing excess adiposity for healthy aging.

  The Norwegian authorities started a nationwide free school fruit program in 2007, implemented in all secondary schools (grades 8–10) and combined schools (grades 1–10) in Norway. The program ended in 2014. This study evaluates the effect of ending the nationwide free school fruit program on the consumption of fruit, vegetables, and unhealthy snacks among Norwegian sixth and seventh graders. The study sample consists of pupils at 18 schools that participated in all data collections in the Fruits and Vegetables Make the Marks project (FVMM), initiated in 2001, with new data collections in 2008 and 2018. Four of the schools were combined schools, therefore children in sixth and seventh grade at these schools received free fruit in 2008 (intervention schools), and fourteen schools did not (control schools). Between 2008 and 2018, pupils at the intervention schools ate a lower proportion of fruits and vegetables per school week, and the consumption of unhealthy snacks increased compared to the control schools. Completion of the free fruit program was not significantly different for boys and girls, or low and high parental education. The results indicate that the end of the free school fruit program resulted in less healthy eating habits among children.

  The NOURISHING database is a repository of more than 1000 verified nutrition anddiet-related governmental policy actions currently in effect globally. The database isa unique and rich data source on governmental policy actions with a potential fordeveloping tools that capture the overall policy efforts in a country, identify policygaps, and enable cross-national comparisons. Policy actions from a sample of fiveEuropean countries have been benchmarked against aspirational standards using theNOURISHING benchmarking tool. This paper presents the results of the pilot testingfrom the benchmarking process for the construction of the NOURISHING policyindex. The development of the index was guided by existing tools for developingcomposite indicators. The findings from the pilot test indicate that the NOURISHINGpolicy index can identify both policy gaps and cross-national policy differences. Theseresults demonstrate that the policy index merits testing on a larger sample to identifypotential refinements.

  Background and objectives: Public procurement may accelerate the transition to a more sustainable food system by stimulating access to, and demand for, healthy foods with a lower impact on climate and the environment. Currently, there is limited information about the potential use of public procurement as a tool for healthier and more sustainable diets. First, this study aimed to investigate the nutritional quality and environmental impact of food procurement among upper secondary schools in former Viken County in Norway. Second, we aimed to examine the nutritional and environmental impacts of changes in food procurement after one year of intervention for healthier and more sustainable school canteens, organised by former Viken County in collaboration with the guidance service “Matvalget”. Methods: The main sample included procurement data for a fiscal quarter from 38 upper secondary schools, including data from 36 and 17 schools in 2022 and in 2023, respectively. The subsample analysis included baseline and follow-up data after one year of intervention, for 8 schools that participated in the intervention and 7 control schools without intervention. After extensive data processing, data was imported to the nutritional calculation system Kostberegningssystemet (KBS, version AE-22) at the University of Oslo. KBS enables estimation of nutritional content and environmental impact of the procurement data, including greenhouse gas emissions, eutrophication of freshwater and marine waters, soil acidification, land- and water use. Results: Preliminary results for the main sample show that soft drinks contributed to the largest share (in grams) of the procurement, followed by milk (including flavoured milk), fresh/frozen vegetables, and bread. The preliminary subsample analysis found a lower share of soft drinks, and increased shares of milk, white unprocessed meat and vegetable oils in the intervention schools compared to control schools. Further results from the analysis will be available in spring 2024. Discussion: The preliminary subsample analysis suggests nutritional improvements in the procurement pattern for intervention schools, however further investigation is required to conclude. Public procurement data from electronic invoices require extensive data processing before analysis to be utilized as a tool for healthier and sustainable diets and there is a need to standardize input to increase usability of the data.
