  Objective Adults operated for tetralogy of Fallot (TOF) have high risk of ventricular arrhythmias (VA). QRS duration >180 ms is an established risk factor for VA. We aimed to investigate heart function, prevalence of arrhythmias and sex differences in patients with TOF at long-term follow-up. Methods We included TOF-operated patients≥18 years from our centre’s registry. We reviewed medical records and the most recent echocardiographic exam. VA was recorded on ECGs, 24-hour Holter registrations and from implantable cardioverter defibrillator. Results We included 148 patients (age 37±10 years). Left ventricular global longitudinal strain (LV GLS, −15.8±3.1% vs −18.8±3.2%, p=0.001) and right ventricular (RV) GLS (−15.8±3.9% vs −19.1±4.1%, p=0.001) were lower in men at all ages compared with women. Higher RV D1 (4.3±0.5 cm vs 4.6±0.6 cm, p=0.01), lower ejection fraction (55%±8% vs 50%±9%, p=0.02), lower RV GLS (−18.1±4.0 ms vs −16.1±4.8 ms, p=0.04) and N-terminal pro-brain natriuretic peptide (NT-proBNP) over reference range (n=27 (23%) vs n=8 (77%), p<0.001) were associated with higher incidence of VA. QRS duration was longer in men (151±30 ms vs 128±25 ms, p<0.001). No patients had QRS duration >180 ms. QRS duration did not differ in those with and without VA (143±32 ms vs 137±28 ms, p=0.06). Conclusions Our results confirmed reduced RV function in adults operated for TOF. Male patients had impaired LV and RV function expressed by lower LV and RV GLS values at all ages. Reduced cardiac function and elevated NT-proBNP were associated with higher incidence of VA and may be important in risk assessment.

  Objective: Patients with tetralogy of Fallot (TOF) have high survival rates 30 years after surgical repair. Many patients experience pregnancy; however, the effects of pregnancy on the long-term cardiovascular outcome are not well known. We investigated the association of pregnancy and cardiac function with occurrence of ventricular arrhythmia (VA) in women with TOF. Methods: We recruited 80 women with repaired TOF from the national database. Holter monitoring or implanted devices detected VA, defined as non-sustained or sustained ventricular tachycardia or aborted cardiac arrest. All patients underwent echocardiography. Blood tests included NT-proBNP (N-terminal pro-brain natriuretic peptide). Results: 55 (69%) women had experienced pregnancy. Mean age was lower in nulliparous compared with those with children (30±9 vs 40±9, p<0.01).VA had occurred in 17 (21%) women. Prevalence of VA was higher in women who had experienced pregnancy (n=16, 94%) compared with nulliparous (n=1, 6%) (p=0.02), also when adjusted for age (OR 12.9 (95% CI 1.5 to 113.2), p=0.02).Right ventricular mechanical dispersion was more pronounced in patients with VA (50±8 ms vs 39±14 ms, p=0.01, age-adjusted OR 2.1 (95% CI 1.3 to 7.5), p=0.01). NT-proBNP was also a marker of VA (211 ng/L (127 to 836) vs 139 ng/L (30 to 465), p=0.007). NT-proBNP >321 ng/L (normal values <170 ng/L) detected women with VA (p=0.019), also independent of age (OR 7.2 (95% CI 1.7 to 30.1), p=0.007). Conclusion: Pregnancy was associated with higher prevalence of VA among women with TOF. Right ventricular mechanical dispersion and NT-proBNP were age-independent markers of VA.   In patients operated with atrial switch for transposition of the great arteries (TGA), the left ventricle (LV) supports the pulmonary circulation and is thus pressure unloaded. Evaluation of LV function in this setting is of importance, as LV functional abnormalities have been documented and might contribute to development of symptoms. The ventricular contraction pattern in 14 Senning-operated TGA patients and 14 healthy controls was studied using tissue Doppler and magnetic resonance imaging. In the subpulmonary LV free wall, longitudinal strain was greater than circumferential strain (-23.6 +/- 3.6% vs. -19.1 +/- 3.2%, p = 0.002) as in the normal right ventricle (RV) (-30.7 +/- 3.3% vs. -15.8 +/- 1.3%, p < 0.001), but opposite to findings in the normal LV (-16.5 +/- 1.7% vs. -25.7 +/- 3.1%, p < 0.001). Subpulmonary strain and strain rate values were intermediate between those in the normal LV and RV. Ventricular free-wall torsion was reduced in the subpulmonary LV compared with both the normal LV (5.7 +/- 3.2 degrees vs. 16.7 +/- 5.6 degrees , p < 0.001) and RV (5.7 +/- 3.2 degrees vs. 11.4 +/- 2.6 degrees , p < 0.05). Furthermore, early diastolic filling of the subpulmonary LV differed from that of the normal LV. The subpulmonary LV displayed predominantly longitudinal shortening, as did its functional counterpart, the normal RV. However, the degree and rate of both longitudinal and circumferential shortening were intermediate between those of the normal LV and RV. This could represent a partial adaptation to the reduced pressure load. Decreased ventricular torsion and diastolic abnormalities might indicate subclinical ventricular dysfunction.

  Aims: To register mortality and causes of death in patients with congenital heart defects (CHDs). Methods: Prospective population-based observational study. Results: 553 infants with CHD (1.1% of live born) were observed for 1-22 y (median 10 7/12 y). Sixty-four died (11.6%), of whom 32 (50%) died during the first 4 wk, and 51 (79.7%) during the first year of life. Of the total neonatal deaths in the population (3 per 1000), CHDs occurred in 21.5%. Mortality for children with CHDs was not significantly different between the cohorts born in 1982-1991 and 1992-2002, for either neonatal deaths or deaths later on (p>0.05). Out of 170 patients in whom therapeutic procedures (surgery, catheter interventions) were undertaken, 34 (20%) died. Nine cases (1.6%) died with unrecognized CHDs; seven of these on the first day of life with severe extracardiac malformations. In 50 (78.1%) cases, death was judged to be caused directly or indirectly from the CHD, and in 14 (21.9%) from extracardiac malformations or other conditions.Conclusion: CHDs occur in a substantial number of neonatal deaths. Most deaths are caused by cardiac insufficiency. The mortality rate remained unchanged.

  AIM OF STUDY: To evaluate all patients with a diagnosis of Tetralogy of Fallot (TOF) and related congenital malformations (Double Outlet Right Ventricle of TOF type, Pulmonary Atresia of TOF type, TOF with absent Pulmonary Valve, TOF with AV-canal) surgically treated in our Department during the period 1979 through 2007. METHODS: Patient records and the database of the department were evaluated, including earlier procedures performed in all patients. The official death registry of Norway was used for follow up RESULTS: A total of 420 patients were included in the study. There were 22 early and 23 late deaths. 399 patients have had a reparative procedure performed, with an EM of 11 (2,75 %). 151 cases has undergone a second repair, 47 have had a third operation performed, ten patients have been operated for a 4th time. Only three patients have been operated on with sternotomy for the 5th time. Mortality in this group was 15 early and 14 late. Our current policy is to do a primary repair at any age if the pulmonary valve is of adequate size. CONCLUSION: Surgical treatment of the Tetralogy of Fallot and related congenital cardiac malformations has been performed for a long time with good results in our Department. In this cohort of patients almost 40% required additional procedures later on, in some cases as many as four additional surgeries. Reoperations carries a low risk in the second and third procedure, but may be more hazardous doing sternotomy for the fourth or fifth time.

  Abstract Postpneumonectomy syndrome refers to a condition where mediastinal shift and rotation gives rise to airway obstruction. The syndrome is more common after right pneumonectomy and the shift may be more pronounced in children than in adults probably because the softer structures in children. (1,2). We report two infants who underwent right pneumonectomy in infancy and developed postpneumonectomy syndrome with obstruction of the left main bronchus causing sever airway obstruction in one patient and severe gastrointestinal reflux due to a displaced and grossly dilated oesophagus in the other patient. Both patients were operated with implantation of expandable breast prosthesis. In one of the patients the volume of the prosthesis had to be reduced twice in the early postoperative period due to compression of mediastinal structures. Both patients are discharged from hospital and presently without symptoms.

  Abstract Background: The aim of this study was to evaluate patients with a diagnosis of Tetralogy of Fallot (TOF) and related congenital malformations undergoing surgery in our Department since the start of treating patients with tetralogy of Fallot in the early fifties. Methods: Operative protocols, patient records and the database of the department were evaluated for more than fifty years. The official death registry of Norway was used for follow up. The follow up is 99.6%. Results: A total of 627 patients were found. Of these, 571 could be identified for follow-up and are included in the study by having a surgical procedure during the inclusion time period. There were a total of 41 early and 30 late deaths. 264 patients underwent some form of palliative procedure as their first treatment. 541 patients had a reparative procedure performed, with an EM of 31 (5.7 %). Conclusions: Surgical treatment of the Tetralogy of Fallot and related congenital cardiac malformations has good long term prognosis. In this cohort of patients more than one third (185 out of 541) required additional procedures later on, in some cases as many as four additional surgeries. • Palliative procedures followed by repair does not increase risk of re-repair • There are no differences between transatrial vs. transventricular repair on survival or re-repair • Any transannular procedure increases the risk of re-repair, but does not influence long time survival • The hazard of death and/or re-repair is almost linear up to fifty years
