  Introduction During the last quarter of a century, new surgical techniques in neonates have been introduced, and neonatal intensive care has developed. Few studies have explored the implementation of new techniques and if outcomes in neonates undergoing gastrointestinal surgery have improved in the last decades. Therefore, this study aimed to investigate possible changes in postoperative outcomes and surgical techniques in all neonates operated for congenital duodenal obstruction (CDO) 1995 to 2020 in Norway. Material and Methods This is a national multicenter retrospective study of all neonates undergoing surgery for CDO in Norway from 1995 to 2020. Results from three periods (1995–2003, 2004–2012, and 2013–2020) were compared. The study was approved by the local data protection officers (2020/13386) and (2020/15125). Results We included 186 patients: 41 in period 1 (1995–2003), 83 in period 2 (2004–2012), and 62 in period 3 (2013–2020). Seventy (38%) neonates had Down syndrome and 104 (62%) had additional malformations/disorders. Birth weight, gender, frequency of Down syndrome, and other malformations/disorders did not differ between the three periods. We observed an increased rate of prenatal diagnosis throughout the study period (p < 0.001). The only change in surgical technique was the increased use of transanastomotic feeding tubes (p < 0.001). Length of stay, postoperative complication rate, days with parenteral nutrition, and 30-day mortality rate were stable over time. Conclusion Perioperative treatment and postoperative outcomes in neonates with CDO have been surprisingly unchanged during the last quarter of a century. Only an increased rate of prenatal diagnosis and more frequent use of transanastomotic feeding tubes were observed.   The rapid spread of COVID-19 had a tremendous impact on healthcare systems and the medical community, as resources were diverted towards priority setting. In many specialties, both non-urgent and urgent activities were reduced or postponed1 . Although children were marginally affected by the pandemic, paediatric surgical services were significantly affected2 . Moreover, the entire medical educational system, and particularly surgical training, was compromised as was the well-being of trainees3 . The changes and challenges imposed by the pandemic on trainee education are largely under-reported. This study aimed to assess the effects of the pandemic on paediatric surgical resident activity and training throughout Europe. An anonymous survey was conducted among paediatric surgery centres across Europe between November 2020 and January 2021 (Table S1). The questionnaire consisted of 30 closed-end and multiple-choice questions, divided into three sections: general information; impact of COVID-19 on overall department activity; and influence on trainee-specific activities and/or teaching. A total of 343 surgeons answered, from 68 centres across 10 European countries. Responders originated mostly from high-volume centres (n = 57, 86 per cent). Since the pandemic outbreak, 75 per cent of the responding centres had been treating patients with COVID-19 only as an emergency. Ten centres (15 per cent) were COVID-19-free hospitals. Redeployment of surgeons towards COVID-dedicated units was observed in 26 centres (38 per cent). The activities mostly affected by the pandemic were operations and outpatient visits, whereas ward rounds were maintained overall. Elective procedures were significantly reduced in 34 centres (50 per cent), and completely interrupted in 19 (28 per cent), compared with other types of surgery (P = 0.04) (Fig. 1a). A significant difference between centres with few and many trainees was observed for emergency (P = 0.001), neonatal (P < 0.001), and oncological surgery (P < 0.001), whereas no variation was found in elective surgery (P = 0.5) (Fig. 1b). Multidisciplinary meetings and teaching were fully interrupted in 31 centres (46 per cent) and reduced in 24 (35 per cent); 82 per cent of centres shifted towards virtual webinars. Only 12 per cent of centres provided constant access to surgical simulators. In contrast, clinical and basic research was preserved; 29 centres (43 per cent) declared the same activity as before the pandemic, and 37 reported having the same or an increased number of scientific publications (54 per cent). Pandemic-related restrictions limiting non-emergency operations resulted in decreased surgical volume for trainees4 , inevitably raising concerns about the learning curve and completion of educational requirements

  We hypothesized that resuscitation with 100% O(2) compared with 21% O(2) is detrimental to pulmonary tissue. The pulmonary injury was assessed by matrix metalloproteinase (MMP) activity, oxidative stress, IL-8, and histology 2.5 h after resuscitation from a hypoxic state. In pulmonary tissue extracts, MMP activity was analyzed by broad matrix-degrading capacity (total MMP) and zymography. MMP-2 mRNA expression was evaluated by quantitative real-time PCR. Total endogenous antioxidant capacity was measured by the oxygen radical absorbance capacity (ORAC) assay, and IL-8 was analyzed by ELISA technique. In bronchoalveolar lavage (BAL) fluid, MMPs were analyzed by zymography. In pulmonary tissue, pro- and active MMP-2 levels were increased in piglets that were resuscitated with 100% O(2) compared with 21% O(2). Pro-MMP-9, total MMP activity, and MMP-2 mRNA levels were significantly increased in resuscitated piglets compared with baseline. Net gelatinolytic activity increased in submucosa and blood vessels after 100% O(2) and only in the blood vessels after 21% O(2). Compared with baseline, ORAC values were considerably lowered in the resuscitated piglets and significantly reduced in the 100% O(2) versus 21% O(2) group. In BAL fluid, both pro-MMP-9 and pro-MMP-2 increased 2-fold in the 100% O(2) group compared with 21% O(2). Moreover, IL-8 concentration increased significantly in piglets that were resuscitated with 100% O(2) compared with 21% O(2), suggesting a marked proinflammatory response in the pulmonary tissue. Altogether, these data strongly suggest that caution must be taken when applying pure O(2) to the newborn infant.

  Fifteen one-year old infants with oesophageal atresia and their parents were used to demonstrate how the Parent-Child Early Relational Assessment (PCERA) can be used to explore the mother-child interaction in a clinical sample of children. The infants and their mothers were videotaped for 5-min episodes during feeding and free play, and the quality of the mother-infant interactions was assessed by PCERA. The results showed areas of strength regarding most of the maternal, infant and dyadic components. There was concern about maternal positive physical contact, maternal quality of verbalizations to or about the child, maternal social initiative, infant's communicative competence and infant's visual contact with mother. This concern of social proximity between infants and their mothers was most prominent in the feeding situation. The present study indicates that oesophageal atresia in infants may constitute stressors impinging on the parent-infant relationship. PCERA was found to be an adequate assessment tool with acceptable reliability to evaluate the mother-infant interaction

  Background Perinatal asphyxia is associated with cardiac dysfunction, and it is important to prevent further tissue injury during resuscitation. There is increasing evidence that myocardial matrix metalloproteinases (MMPs) are involved in myocardial hypoxaemia-reoxygenation injury. Objective To assess MMPs and antioxidant capacity in newborn pigs after global ischaemia and subsequent resuscitation with ambient air or 100% O2 at different PaCO2-levels. Methods Newborn pigs (12-36 h of age) were resuscitated for 30 min by ventilation with 21% or 100% O2 at different PaCO2 levels after a hypoxic insult, and thereafter observed for 150 min. In myocardial tissue extracts, MMPs were analyzed by gelatin zymography and broad matrix-degrading capacity (total MMP). Total endogenous antioxidant capacity in myocardial tissue extracts was measured by the oxygen radical absorbance capacity (ORAC) assay. Results Matrix metalloproteinase-2 more than doubled from baseline values (P < 0·001), and was higher in piglets resuscitated with 100% O2 than with ambient air (P = 0·012). The ORAC value was considerably decreased (P < 0·001). In piglets with elevated PaCO2, total MMP-activity in the right ventricle was more increased than in the left ventricle (P = 0·008). In the left ventricle, total MMPactivity was higher in the piglets with low PaCO2 than in the piglets with elevated PaCO2 (P = 0·013). Conclusion In hypoxaemia-reoxygenation injury the MMP-2 level was highly increased and was most elevated in the piglets resuscitated with 100% O2. Antioxidant capacity was considerably decreased. Assessed by total MMP-activity, elevated PaCO2 during resuscitation might protect the left ventricle, and probably increase right ventricle injury of the myocardium.

Parenting stress is an important factor influencing parenting behavior and parent-child interaction. This kind of stress may arise from child, parent, and environmental characteristics. Little is known about parental stress around and after the birth of a baby with a surgical anomaly. Aim of the study: To assess psychological stress in parents of one year old children with a severe congenital malformation and parents of children with minor congenital malformations compared to psychological stress in parents of children in the general population. Material and methods: As part of a prospective, longitudinal follow up study of children with esophageal atresia we assessed psychological stress in their parents. 18 one year old children with esophageal atresia, 10 children matched on age and gender with minor urological problems, and 100 1-2 year old children in the general population and their parents were included. The study was a cross-sectional questionnaire study. Standardized psychometric self report instruments (GHQ 30, STAI-X1 and SPSQ) were used, assessing depression, anxiety and parenting stress. Child medical data were registered from medical reports. Results: Multidimensional outcome variables will be tested, searching for predictors of parent stress among children with surgical congenital anomalies and normal children. The results will be presented.

  Parental psychological stress in children with nutritional- and eating disorders. Aim of the study. Registration of parental psychological stress before and after gastrostomy insertion in children with nutritional- and eating disorders. Background. Gastrostomy is a method of establishing enteral feeding in children with major nutritional- and eating disorders. These children often have major disabilities and serious diseases which can put their parents under major psychological distress. Parents have reported better quality of life after gastrostomy insertion when asked. We wanted to test this in a follow-up study. Methods. N mothers, and nn fathers, were assessed before and after (3-12 months) their child had had a gastrostomy insertion. The assessment included General Health Questionnaire-30 (GHQ-30), State Anxiety Inventory X1-12, State Anxiety Inventory X2-20 and Impact of Event Scale. During a semi-structured interview we also asked how the parents considered the results of the gastrostomy insertion. Results. So far parents of 20 children with median age nn (range 0,67-10,68 years) have been included. Preliminary results show that the children suffer from a variety of diseases including neurological impairment, heart disease and others. Results of the children’s situation and of the assessments of the parents will be presented.

A serious medical illness that occurs during infancy creates adversities in the infant and represents emotional responses by parents, being challenging to the parent-child interaction. It is essential to use a reliable and valid standardized instrument as PCERA and CARE-index in assessing this parent-child relationship. Esophageal atresia (EA) is a congenital anomaly requiring neonatal surgery. Typical symptoms are frothing and bubbling of saliva, coughing, and respiratory distress. Feeding exacerbate these symptoms. Invasive procedures may be necessary to prevent nutritional problems. Little is known about long-term psychosocial function of children with EA, and the emotional response the parent brings into the parent child interaction and relationship. Aim of the study was to compare the parent-child interaction in infants with EA with infants with minor urological diseases submitted to the surgical ward using CARE-index and PCERA. The sample consists of 39 one year old infants with EA born in 1999-2003, and their mothers and fathers. The comparison group consists of 10 infants healthy at birth, submitted at the surgery unit, matched for age and gender, and their parents. GHQ and STAI assessing parent well being, depression and anxiety were used. Registration of medical data was done prospectively from first admission. The dyadic profiles of the two groups on the CARE-index will be compared with those on the PCERA.
