  Objectives We compared Monoamine oxidase B (MAO-B) – and dopamine agonist (DA) monotherapy patients with respect to survival, considering gender, age, first prescriber’s specialty and relevant co-morbidity, and compared their specialist health care contacts and hospitalizations. Methods With data from health registries, we considered 21,047 patients without redemptions for MAO-B, DA or levodopa 6 months prior to their first MAO-B or DA redemption in 2006 and followed them throughout 2016. We considered Cox proportional hazard regression models for comparing the risk of death among MAO-B and DA monotherapy patients. Results MAO-B-users had a higher mortality than DA-users, [HR: 1.587, 95% CI: 1.056; 2.384] for patients under 74 years. There was an increased mortality risk with increasing age, women had lower risk than men and previous diabetes-, antihypertensive-, and cardiac drug users had higher risk compared to patients without such history. Previous use of hypothyroid drugs and having a specialist as first prescriber were not significant risk factors. Among patients without hospitalizations 13.7% died, while among patients who spent at least one night in hospital 36.73% died. The median duration of a hospitalization among those who died and not were 17.5 and 7 days. Among the small proportion with specialist health care contacts circulatory- and respiratory-system diseases were the most frequent cause of contact. Conclusions DAs were most frequently given when initiating Parkinson’s treatment. DA-users had a lower mortality risk compared to MAO-B-users and less specialist health care contact.

  Objective. To investigate the use of dopamine agonists and monoamine oxidase type B (MAO-B) inhibitors in the Norwegian population, between 1 July 2006 and 31 December 2016. Our primary endpoint was time until need for levodopa among new monotherapy users of dopamine agonists and MAO-B inhibitors. Methods. A prospective cohort study including all patients, aged 50 years or above, who had at least one prescription for a dopamine agonist or a MAO-B inhibitor dispensed in the study period. We used data from the Norwegian Prescription Database (NorPD). As we wished to focus on new Parkinson patients, we excluded patients who had levodopa dispensed less than 180 days prior to their first dopamine agonist or MAO-B inhibitor redemption. We explored the demographics and the time until monotherapy was insufficient treatment (defined as need for levodopa prescription). Results. We included 22958 new monotherapy users. Of these, 22108 used dopamine agonists and 850 used MAO-B inhibitors. The mean number of days until the first prescription of levodopa was dispensed was higher among the dopamine agonist users (621 days) compared to the MAO-B inhibitor users (352 days). The proportion of dopamine agonist users who started levodopa treatment during the study period was less than 7%, while the corresponding proportion of MAO-B inhibitor users was almost 59%. Conclusions. We found that new dopamine agonist users had a much greater delay in the need for levodopa than new MAO-B inhibitor users. It seems to be beneficial to initiate treatment with dopamine agonists when starting pharmacological treatment for new Parkinson patients.

  Purpose To investigate the comparative effectiveness of dopamine agonists and monoamine oxidase type-B (MAO-B) inhibitors available for treatment of Parkinson’s disease. Methods We performed a systematic literature search identifying randomized controlled trials investigating 4 dopamine agonists (cabergoline, pramipexole, ropinirole, rotigotine) and 3 MAO-B inhibitors (selegiline, rasagiline, safinamide) for Parkinson’s disease. We extracted and pooled data from included clinical trials in a joint model allowing both direct and indirect comparison of the seven drugs. We considered dopamine agonists and MAO-B inhibitors given as monotherapy or in combination with levodopa. Selected endpoints were change in the Unified Parkinson’s Disease Rating Scale (UPDRS) score, serious adverse events and withdrawals. We estimated the relative effectiveness of each dopamine agonist and MAO-B inhibitor versus comparator drug. Results Altogether, 79 publications were included in the analysis. We found all the investigated drugs to be effective compared with placebo when given as monotherapy except safinamide. When considering combination treatment, the estimated relative effects of selegiline, pramipexole, ropinirole, rotigotine, cabergoline, rasagiline and safinamide were 2.316 (1.819, 2.951), 2.091 (1.889, 2.317), 2.037 (1.804, 2.294), 1.912 (1.716, 2.129), 1.664 (1.113, 2.418), 1.584 (1.379, 1.820) and 1.179 (1.031, 1.352), respectively, compared with joint placebo and levodopa treatment. Conclusions Dopamine agonists were found to be effective as treatment for Parkinson’s disease, both when given as monotherapy and in combination with levodopa. Selegiline and rasagiline were also found to be effective for treating Parkinson’s disease, and selegiline was the best option in combination with levodopa among all the drugs investigated.

  Aims To the best of our knowledge, there are no systematic reviews or meta‐analyses that compare rasagiline, selegiline and safinamide. Therefore, we aimed to perform a drug class review comparing all available monoamine oxidase type B (MAO‐B) inhibitors in a multiple treatment comparison. Methods We performed a systematic literature search to identify randomized controlled trials assessing the efficacy of MAO‐B inhibitors in patients with Parkinson's disease. MAO‐B inhibitors were evaluated either as monotherapy or in combination with levodopa or dopamine agonists. Endpoints of interest were change in the Unified Parkinson's Disease Rating Scale (UPDRS) score and serious adverse events. We estimated the relative effect of each MAO‐B inhibitor versus the comparator drug by creating three networks of direct and indirect comparisons. For each of the networks, we considered a joint model. Results The systematic literature search and study selection process identified 27 publications eligible for our three network analyses. We found the relative effects of rasagiline, safinamide and selegiline treatment given alone and compared to placebo in a model without explanatory variables to be 1.560 (1.409, 1.734), 1.449 (0.873, 2.413) and 1.532 (1.337, 1.757) respectively. We also found all MAO‐B inhibitors to be efficient when given together with levodopa. When ranking the MAO‐B inhibitors given in combination with levodopa, selegiline was the most effective and rasagiline was the second best. Conclusions All of the included MAO‐B inhibitors were effective compared to placebo when given as monotherapy. Combination therapy with MAO‐B inhibitors and levodopa showed that all three MAO‐B inhibitors were effective compared to placebo, but selegiline was the most effective drug.

  Rheumatoid arthritis patients have been treated with disease modifying anti-rheumatic drugs (DMARDs) and the newer biologic drugs. We sought to compare and rank the biologics with respect to efficacy. We performed a literature search identifying 54 publications encompassing 9 biologics. We conducted a multiple treatment comparison regression analysis letting the number experiencing a 50% improvement on the ACR score be dependent upon dose level and disease duration for assessing the comparable relative effect between biologics and placebo or DMARD. The analysis embraced all treatment and comparator arms over all publications. Hence, all measured effects of any biologic agent contributed to the comparison of all biologic agents relative to each other either given alone or combined with DMARD. We found the drug effect to be dependent on dose level, but not on disease duration, and the impact of a high versus low dose level was the same for all drugs (higher doses indicated a higher frequency of ACR50 scores). The ranking of the drugs when given without DMARD was certolizumab (ranked highest), etanercept, tocilizumab/ abatacept and adalimumab. The ranking of the drugs when given with DMARD was certolizumab (ranked highest), tocilizumab, anakinra, rituximab, golimumab/ infliximab/ abatacept, adalimumab/ etanercept. Still, all drugs were effective. All biologic agents were effective compared to placebo, with certolizumab the most effective and adalimumab (without DMARD treatment) and adalimumab/ etanercept (combined with DMARD treatment) the least effective. The drugs were in general more effective, except for etanercept, when given together with DMARDs.

  Background While prophylactic human papilloma virus (HPV) vaccination is considered effective in young girls, it is unclear whether a catch-up vaccination of older girls would be beneficial. We, therefore, aimed to examine the potential health impact of a HPV catch-up vaccination of girls who were too old at the time of vaccine introduction, hence aged 16 and older. Methods We systematically searched the literature for randomized clinical trials (RCTs) that examined the effect of HPV vaccines on overall mortality, cancer mortality and incidence, high-grade cervical intraepithelial neoplasia grade 2 and higher (CIN2+), vulvar intraepithelial neoplasia (VIN) and vaginal intraepithelial neoplasia (VaIN) grade 2 and higher lesions (VIN2+ and VaIN2+, respectively) genital warts (condyloma). We considered all lesions and those associated with HPV type(s) included in the vaccines. RCTs reporting on serious adverse events were also eligible. Selected publications were assessed for potential risk of bias, and we ascertained the overall quality of the evidence for each outcome using Grading of Recommendations Assessment, Development and Evaluation (GRADE). Meta-analyses were performed, assuming both random and fixed effects, to estimate risk ratios (RR) and corresponding 95% confidence intervals (CI), using intention-to-treat and per-protocol populations. Results We included 46 publications reporting on 13 RCTs. Most of the RCTs had a maximum follow-up period of four years. We identified no RCT reporting on the effect of HPV catch vaccination on overall and cancer related mortality, and on cervical cancer incidence. We found a borderline protective effect of a HPV catch-up vaccination on all CIN2+, with a pooled RR of 0.80 (95% CI: 0.62-1.02) for a follow-up period of 4 years. A HPV catch-up vaccination was associated with a reduction in VIN2+ and VaIN2+ lesions, and condyloma. No difference in risk of serious adverse events was seen in vaccinated participants versus unvaccinated women (pooled RR of 0.99 (0.91-1.08)). Conclusions This systematic review indicates that a HPV catch-up vaccination could be beneficial, however the long-term effect of such a vaccination, and its effect on cervical cancer incidence and mortality is still unclear.

  Background In Norway, pap smear screening target women aged 25–69 years on a triennial basis. The introduction of human papillomavirus (HPV) mass immunization in 2009 raises questions regarding the cost-saving future changes to current screening strategies. Methods We calibrated a dynamic HPV transmission model to Norwegian data and assessed the impact of changing screening 20 or 30 years after vaccine introduction, assuming 60 or 90% vaccination coverage. Screening compliance among vaccinated women was assumed at 80 or 50%. Strategies considered: (i) 5-yearly screening of women of 25–69 years, (ii) 3-yearly screening of women of 30–69 years and (iii) 3-yearly screening of women of 25–59 years. Results Greatest health gains were accomplished by ensuring a high vaccine uptake. In 2060, cervical cancer incidence was reduced by an estimated 36–57% compared with that of no vaccination. Stopping screening at the age of 60 years, excluding opportunistic screening, increased cervical cancer incidence by 3% (2060) compared with maintaining the current screening strategy, resulting in 1.0–2.4% extra cancers (2010–2060). The 5-yearly screening strategy elevated cervical cancer incidence by 30% resulting in 4.7–11.3% additional cancers. Conclusion High vaccine uptake in the years to come is of primary concern. Screening of young women <30 years remains important, even under the conditions of high vaccine coverage.

  Objective. Perform a systematic review to determine the test performance of HPV mRNA testing compared to DNA testing using CIN2+ as the target condition. Methods. We searched bibliographic databases (MEDLINE, EMBASE and Cochrane Library) from January 1996 through August 2010 using a predefined search strategy. The reference standard used to diagnose precancerous lesions was histologically confirmed cervical intraepithelial neoplasia 2+ (CIN2+). Two reviewers independently assessed study eligibility, extracted data, and assessed risk of bias. Sensitivity, specificity, positive and negative likelihood ratios and diagnostic odds ratios were calculated for each study. In addition, we fitted a series of summary receiver operating characteristics (SROC) curves. A subgroup analysis was performed according to specific inclusion covariates. Results. Out of 3179 potentially relevant citations, 12 publications (11 studies) met our inclusion criteria. The included studies were of varying methodological quality, and were predominately performed in a secondary screening setting. Eight studies investigated the performance of the PreTect Proofer/NucliSENS EasyQ, two studies investigated the performance of the APTIMA assay and one study investigated both mRNA tests on the same patient samples. Due to few studies and considerable clinical heterogeneity, pooling of data was not possible. Instead, we compiled a ‘best evidence synthesis’ for E6/E7 mRNA HPV testing. Sensitivities ranged from 0.41 to 0.86 and from 0.90 to 0.95 for the PreTect Proofer/Easy Q and APTIMA assay, respectively. Specificities ranged from 0.63 to 0.97 and from 0.42 to 0.61 for the PreTect Proofer/Easy Q and APTIMA assay, respectively. The SROC curves for both mRNA tests were to the left of the diagonal and the APTIMA assay performed closest to the DNA tests. Conclusion. The review suggests that mRNA tests have diagnostic relevance, but additional studies and economic evaluations must be conducted in order to make a solid conclusion regarding the clinical applicability of HPV mRNA testing.

  1-page key messages Due to a high risk of thromboembolism in patients undergoing major orthopaedic surgery it has become standard practice to give thromboprophylactic treatment to these patients. Pharmaceutical interventions with or without addition of mechanical methods are recommended. This project, commissioned by Helse Bergen HF Ortopedisk klinikk, examined thromboprophylaxis with rivaroxaban or dabigatran compared with low-molecular weight heparins (LMWH, i.e. enoxaparin and dalteparin) with regard to efficacy, safety and cost-effectiveness in patients undergoing elective total hip or knee replacement surgery. We conducted a systematic review of the literature and made cost-effectiveness analyses based on a model that calculated quality-adjusted life years and life time costs. The main findings were that: •We did not find statistically significant differences between dabigatran and enoxaparin for mortality, pulmonary embolism, deep vein thrombosis or major bleeding. The quality of the evidence ranged from very low to moderate. •For rivaroxaban compared with enoxaparin we found statistically a significant reduction in deep vein thrombosis, but also a trend towards increased risk of major bleeding. For mortality and pulmonary embolism there were no statistically significant differences between treatments. The quality of the evidence ranged from very low to moderate. •Our results indicate a great uncertainty regarding which strategy is the most cost-effective. However, rivaroxaban and enoxaparin had a slightly higher probability of being cost-effective alternatives for patients undergoing total hip or knee replacement, respectively. •The results of our model analysis of the uncertainty surrounding each group of parameters indicated that more research on efficacy data would have the greatest impact on reducing decision uncertainty.

  Bakgrunn: Mellom 20- og 30 000 mennesker har leddgikt i Norge. Dette er en alvorlig kronisk sykdom som varig nedsetter funksjons- og arbeidsevne. Forskning har vist at man kan oppnå god effekt ved å behandle sykdommen tidlig med såkalte biologiske legemidler. Slike legemidler er imidlertid betydelig mer kostbare enn medikamentell behandling med konvensjonelle sykdomsmodifi sernede legemidler, såkalte DMARDs. Kunnskapssenteret har derfor på oppdrag fra Norsk Revmatikerforbund oppsummert helseøkonomiske studier som sammenligner kostnader og effekter av disse behandlingene. Kunnskapssenteret har tidligere søkt etter studier om legemidlenes effekt og sikkerhet og publisert en separat rapport om dette (Kornør H, et al. Rapport fra Kunnskapssenterett nr. 9/2010). Fremgangsmåte: Vi søkte systematisk etter helseøkonomiske evalueringer av biologisk behandling av pasienter med tidlig leddgikt (sykdomsvarighet på 3 år eller kortere) sammenlignet med DMARDS. Vi fant seks studier, som alle omfattet TNF-hemmerne adalimumab, etanercept og infl iximab. Ingen studier med de øvrige biologiske legemidlene ble funnet. Hovedfunn: • Vi fant at: • Bruk av biologiske legemidler kan være kostnadseffektivt ved tidlig leddgikt, dog ikke som førstevalg i behandlingen • Dette er i overensstemmelse med gjeldende retningslinjer for behandling av leddgikt i Norge, som tilsier at minst ett DMARD skal være forsøkt før man begynner med biologiske legemidler • Inklusjon av indirekte kostnader kan ha stor betydning for resultatene • Det er til dels betydelig variasjon i resultatene.

  1-page key messages B ackground Stroke is the third most common cause of death, a major cause of severe disability in Norway and accounts for considerable amounts of healthcare resources. The organization of treatment and rehabilitation of stroke patients may improve functional outcomes and enhance quality of life for individuals with acute stroke. Task requirement Norwegian Directorate of Health’s development group for the preparation of national clinical guideline for stroke has commissioned Norwegian Knowledge Centre for the Health Services to conduct economic evaluations of some central recommendations in the stroke guideline. We evaluated the clinical efficacy and conducted health economic model analyses of stroke unit care compared with stroke unit care followed by early supported discharge or general medical ward care. Main Results Ordinary stroke unit care is associated with probably lower mortality than care in general medical wards, whereas there is possibly little or no difference between the two strategies for moderate or severe sequelae. The results of the meta-analyses of comparison between two different stroke units (with and without early supported discharge) show possibly little or no difference in mortality and dependency with care in stroke unit with early supported discharge. The economic evaluation found that ordinary stroke unit care dominate the care in the general medical ward because it has lower expected cost and higher expected quality-adjusted life years. Stroke unit care followed by early supported discharge reduces lifetime costs and adds quality-adjusted life years compared with ordinary stroke unit care. The sensitivity analyses indicate that stroke unit care followed by early supported discharge most likely is the most cost-effective strategy.

Task requirement The Norwegian Directorate of Health’s development groups for the preparation of national clinical guideline for stroke have commissioned the Norwegian Knowledge Centre for the Health Services to conduct economic evaluations of some central recommendations in the stroke guideline. We evaluated the clinical efficacy and conducted health economic evaluation of: 1. Intravenous thrombolytic treatment of patients with acute stroke (within 3 hours and between 3 to 5 hours after symptom onset) in addition to standard treatment compared to treatment without thrombolysis 2. Pharmacological secondary prevention of stroke Antiplatelet therapy: acetylsalicylic acid (ASA) combined with slow-release dipyridamole compared with ASA monotherapy Antiplatelet therapy: ASA combined with slow-release dipyridamole compared with clopidogrel monotherapy Anticoagulation therapy with warfarin compared with ASA for prophylaxis of stroke in patients with atrial fibrillation Main Results Thrombolytic treatment within 3 hours after stoke reduces lifetime costs and adds quality-adjusted life years (QALYs) compared with standard treatment without thrombolysis for selected stroke patients. Thrombolysis given between 3 and 5 hours after stroke is cost-effective compared to no thrombolytic treatment. However, the choice of thrombolysis in this time interval should also be carefully considered from an ethical perspective, because it leads to shorter life expectancy relative to no thrombolytic treatment. The combination of ASA and extended-release dipyridamole increases QALYs and reduces lifetime costs compared with ASA monotherapy in secondary pre-vention of stroke. The use of ASA combined with slow-release dipyridamole for patients of 70 years reduces lifetime costs and adds QALYs compared to clopidogrel for sec-ondary prevention of stroke. Anticoagulation therapy with warfarin has lower expected costs and higher expected QALYs compared with ASA therapy for stroke patients with atrial fibrillation.

  Bakgrunn: Revmatoid artritt (leddgikt) er en kronisk autoimmun sykdom som forårsaker betennelse i leddene. Sykdommen rammer 0,5 % til 1 % av den voksne befolkningen, og medfører store smerter og funksjonshemming. Direkte kostnader knyttet til behandling, samt indirekte kostnader knyttet til yrkesuførhet, er betydelige. Leddgikt behandles med en tverrfaglig tilnærming, hvor sykdomsmodifiserende legemidler (DMARDs) utgjør en viktig komponent. Det anbefalte førstevalget er et eller flere DMARDs. Ved fravær av behandlingseffekt kan et biologisk legemiddel legges til. • Hensikten med denne systematiske oversikten var å undersøke effekt og sikkerhet av biologiske legemidler sammenliknet med DMARDs hos pasienter med tidlig (≤ 3 år) leddgikt. Oppdragsgiver er Norsk Revmatikerforbund, som er opptatt av at leddgiktpasienter får god behandling tidlig i forløpet. Metode: Vi søkte systematisk etter litteratur i EMBASE, Medline (Ovid) og Cochrane Library. I tillegg gikk vi gjennom referanselister i relevante publikasjoner, søkte på relevante nettsider og kontaktet eksperter, berørte firmaer og Norsk Revmatikerforbund. To forskere vurderte sammendrag og fulltekstpublikasjoner for inklusjon uavhengig av hverandre. Resultat: Søkestrategien resulterte i 1560 unike referanser som vi vurderte sammendragene til. Av 144 mulig relevante fulltekstpublikasjoner inkluderte vi 15. De inkluderte publikasjonene omfattet 12 studier som undersøkte effekt og sikkerhet av henholdsvis infliximab, adalimumab, etanercept og abatacept hos leddgiktpasienter med mindre enn 3 års sykdomsvarighet. Konklusjon: Metaanalysene antydet følgende hovedfunn for bruk av biologiske legemidler i kombinasjon med DMARDs: • flere pasienter med foreløpig tilbakegang av sykdomssymptom • verken flere eller færre alvorlige uønskete hendelser • flere pasienter som oppnår 50 % bedring • bedre fysisk funksjon • mindre leddødeleggelse • Metodiske svakheter i de inkluderte studiene gjør at samtlige resultater inneholder en viss grad av usikkerhet.

  Bakgrunn: Dette litteratursøket er utført på oppdrag fra Helseøkonomiforvaltningen HELFO. Bestillingen var en oversikt over studier av den forebyggende effekten av prednisolon og andre steroider, og av verapamil, på cluster hodepine. Metode: Vi søkte systematisk i internasjonale vitenskapelige databaser etter systematiske oversiktsartikler, randomiserte kontrollerte studier (RCTer) og observasjonsstudier som undersøkte nytten av de aktuelle legemidlene. Resultat: Søkeresultatet var 204 unike referanser som vi gikk gjennom sammendragene til. Vi vurderte 17 publikasjoner som relevante for bestillingen. Publikasjonene ble kategorisert etter type legemiddel og studiedesign.
