  Abstract The aim of this study was to explore the usefulness of unilateral, combined unilateral (left + right) and bilateral peak nasal inspiratory flow (PNIF) measurements in assessing the results of nasal septal surgery. Nasal obstruction was recorded subjectively and objectively before and 4 months after nasal septoplasty using a visual analogue scale (VAS) and a PNIF meter. Nasal septoplasty (58 patients) and septoplasty with turbinoplasty (68 patients) were performed on 126 patients (85 males, 41 females) with a mean age of 32.8 years. The results showed a significant improvement in VAS scores, as well as unilateral, combined unilateral and bilateral PNIF values after both septoplasty and septoplasty with turbinoplasty. Septoplasty with turbinoplasty showed better improvement in VAS and PNIF scores than septoplasty alone and this was significant for bilateral PNIF scores. The best unilateral pre- and postoperative correlations between VAS and PNIF measurements were found using the lower of the two unilateral PNIF scores, irrespective of side. In the total material, VAS/PNIF correlations were mostly significant, but weak (all r<0.30). We found VAS and PNIF to be useful instruments in reporting results of surgery. The weak correlations between VAS and PNIF measurements suggest that these subjective and objective instruments may target different aspects of nasal obstruction.

  Abstract Objective: Results from telephone interviews may be needed to supplement those from mailed questionnaire when response rates are inadequate. Our intention was to assess the visual analogue scale ratings used in mailed questionnaires to numerical rating scale scores used in telephone interviews. Methods: Patients scheduled for nasal septal surgery routinely respond to a visual analogue scale of obstruction during the day and at night. In this study they were also asked to verbally rate their sense of obstruction using whole numbers. Results: There was no significant difference in obstruction scores between VAS and NRS recordings. Conclusion: Ratings of nasal obstruction used with NRS in telephone interviews are comparable to those marked on the VAS used in mailed questionnaires.

  OBJECTIVE: Questionnaires are often used to assess the results of nasal septoplasty, but response rates vary widely. The possible bias caused by non-responders was evaluated to determine the validity of questionnaire results. METHODS: Post-operative questionnaires employing visual analogue scales for nasal obstruction were mailed to 182 patients. The 62 non-responders (34.1 per cent) were contacted by telephone, 58 (93.5 per cent) of whom were contactable and responded orally to the questionnaire. RESULTS: Non-responders were younger, but no different from responders with regard to gender, smoking habits or allergies. Post-operative visual analogue scale obstruction scores were slightly, but not statistically, higher in non-responders. However, because non-responders' pre-operative scores were lower, obstruction scores improved less than in responders. The main reason for not responding was forgetfulness. Some would have preferred an electronic version of the questionnaire. CONCLUSION: Although post-operative obstruction scores did not differ between the groups, nasal obstruction scores improved more among responders than non-responders. Thus, low response rates may cause bias

  Dendritic cell (DC) subsets induce different cytokine profiles in naïve T cells. Thus, various DC subsets may determine the differentiation of naïve cells into memory cells with distinct T helper (Th) cell responses. In contrast, recall responses are believed to be mediated by terminally differentiated memory Th cells producing a fixed set of cytokines. Here we show that recall CD4+ T cell responses to mumps virus antigens depend on the type of DC used as antigen-presenting cells (APCs). Plasmacytoid DCs and CD11c+ DCs isolated from peripheral blood of adults and co-cultured under identical conditions with purified autologous CD4+ T cells, both induced antigen-dependent T cell proliferation, but the cytokines produced by the CD4+ T cells differed significantly. While CD11c+ DCs stimulated T cells to produce high levels IL-5 and IFN-gamma, plasmacytoid DCs induced high levels of IL-10 and IFN-gamma, and lower levels of IL-2. Thus, DC subsets appear capable of influencing Th memory immune responses. This may have important implications for modulation of hypersensitivity reactions such as allergy and autoimmune disease.

  Plasmacytoid dendritic cells (P-DCs) induce production of Th2 cytokines in naïve allogeneic CD4+ T cells following incubation with IL-3 and CD40L. It has therefore been suggested that these cells play a role in allergic diseases. In support of this view, we recently demonstrated that P-DCs accumulate in nasal mucosa during allergen-challenged allergic rhinitis. To further investigate the possible role of P-DCs in airway allergy, we examined whether P-DCs can present grass pollen allergens to autologous T cells from allergic individuals, and whether the cytokine response in vitro correlated with cytokine production after allergen challenge in vivo. Peripheral blood was obtained from 3 patients and nasal allergen challenge was performed daily for 7 days in 11 patients, all with a typical history of allergic rhinitis and a strong positive skin prick test. P-DCs and CD4+ T cells were isolated and co-cultured in the presence of purified grass pollen extracts (ALK, Denmark). Using carboxyfluorescein diacetate succinimidyl ester (CFSE) labeling, we found that P-DCs induced extensive antigen-dependent T cell proliferation in primary cultures. The proliferation was associated with prominent production of IL-5, but not TNF-alpha or IFN-gamma. Interestingly, IL-5 became detectable in serum at day 2 through day 7 after challenge in 5 of the 11 patients. Collectively, our results demonstrated that P-DCs are present at sites of pollen exposure, and have the ability to induce a Th2 recall response in sensitized individuals which corresponds to the in vivo response. P-DCs may thus be potential targets for future therapeutic modulation of allergic rhinitis.
