  Bakgrunn. Det er lite kunnskap om behovet for ortopedisk kirurgi hos voksne med cerebral parese. Vi ønsket derfor å undersøke dette problemet nærmere. * Materiale og metode. Materialet består av 37 personer (19 kvinner og 18 menn) med gjennomsnittsalder 39 år. Det var lett fysisk funksjonshemning hos 19 personer og moderat eller betydelig hos 18. Alle ble innkalt til klinisk undersøkelse og røntgenundersøkelse av hofter og rygg. * Resultater. 15 pasienter hadde smerter i rygg eller underekstremiteter, men ingen hadde betydelige eller invalidiserende smerter. Pasienter med moderat eller betydelig funksjonshemning hadde svært redusert bevegelighet i hofte- og kneledd. Røntgenbildene viste subluksasjon eller total luksasjon i hoftene hos seks pasienter, men kun én av dem hadde hoftesmerter. Ortopediske operasjoner var tidligere foretatt hos 23 personer. Ved etterundersøkelsen ble åtte pasienter vurdert til å ha behov for ortopedisk kirurgi for å lindre smerter, redusere kontrakturer eller bedre funksjon. De hyppigste aktuelle prosedyrene var tenotomier i hofter og knær, akillesseneforlengelse og trippelartrodese i fot. * Fortolkning. Regelmessig oppfølging av voksne med cerebral parese er nødvendig for å avdekke problemer som kan bedres ved ortopedisk kirurgi.

  Bakgrunn: Kirurgisk korreksjon av fotdeformiteter hos ambulerende barn med moderat til alvorlig cerebral parese kan være nødvendig for at barnet skal kunne bruke ortoser og få best mulig opptrening etter èn-seanse multilevel kirurgi, såkalt SEMLS. Vi har hos barn som har blitt operert med beinet fotkirurgi samtidig med SEMLS observert mye postoperativ smerte, rehabiliteringsproblemer og psykososiale problemer. Formål: For å undersøke om de dårlige resultatene har sammenheng med fotkirurgien har vi utført en retrospektiv sammenlignende studie ved å se om de kvalitative og kvantitative resultatene etter SEMLS bløtvevskirurgi kombinert med korrigerende beinet fotkirurgi er sammenlignbare med bløtvevskirurgi alene. Pasienter og metoder: Syv barn med diplegisk cerebral parese (fire gutter) som gjennomgikk SEMLS og samtidig beinet fotkirurgi i perioden 2003-2009 ble identifisert for inklusjon. Fem sammenlignbare pasienter operert med SEMLS, men uten fotkorreksjoner, ble matchet og inkludert basert på alder, kjønn, GMFCS-nivå, FMS og typer og antall operasjoner. Alle barna var klassifisert som GMFCS III. Sammenligning av pre-operative gangparametere viste ingen signifikante forskjeller mellom gruppene. Begge gruppene ble behandlet likt etter en streng postoperativ rehabiliteringsprotokoll, inkludert 3-4 uker med intensiv trening på et rehabiliteringssenter. Ganganalyse-parametre registrert preoperativt og ett år postoperativt ble sammenlignet. I tillegg ble data samlet inn ved hjelp av kvalitative, semi-strukturerte intervjuer av de 12 barna og deres foreldre basert på en hovedproblemstilling: På hvilken måte hadde barna og deres foreldre opplevd rehabiliteringsprosessen? Resultater: Barna i kontrollgruppen presterte signifikant bedre i 3 av 9 utvalgte gangparametre ett år postoperativt mens de fot-opererte forbedret kun ett parameter. Fire av syv barn med kombinasjonen av SEMLS og fotkirurgi og ett barn i kontrollgruppen opplevde sterke smerter og hypersensibilitet og utviklet kronisk regionalt smertesyndrom (CRPS) med behov for høyt spesialisert tverrfaglig behandling. Store belastninger ble påført de aktuelle familiene. Fem år etter operasjonen har fortsatt fire av fem barn hypersensitive og smertefull føtter, to har mistet gangevnen, ett barn er ikke lenger selvhjulpen i daglig omsorg og tre er rullestolbundet. Konklusjon: Forsiktighet bør utvises ved å utføre samtidige fotkorreksjoner ved SEMLS hos marginalt ambulante barn med cerebral parese. Postoperative smerter kan hemme rehabiliteringen og forårsake omfattende negative psykososiale effekter på pasientene og deres familier. Vi vil i fremtiden utføre fotkorreksjoner i en egen seanse i forkant av SEMLS.
