MAPINFOTRANS

The purpose is to improve information about medicines for patients with heart failure. We aim to identify what factors are the strongest predictors of adherence and re-admittance rate.

About the project

In MAPINFOTRANS (Many providers, confused information? Coordinated and tailored communication with older patients in hospital-home transitions) we will study how well information is coordinated between providers in hospital and primary care, how well oral and written information are aligned, and whether the information is tailored to the patient's understanding and life situation.

By studying what the doctors say, what is written in the information the patient receives, and by visiting the patients at home, we will try to find out what works best. We will try to identify what has the greatest influence on compliance and re-admittance rate.

Our study is based on elderly patients with heart failure in the transfer phase from hospital to their own home.

Method

In the study, we collected audio recordings, questionnaires, interviews and made home visits.

Elderly patients with heart failure living at home were recruited at hospital admission and followed throughout the trajectory.

We have made audio recordings of conversations between doctor and patient in the ward, upon discharge and at the GP's, focusing on discussions regarding medicines.

Project status

The project began on 1 May, 2021 and will be completed in 2025. The researcher began  1  August, 2021. The PhD students started 15 September, 2021 and 1 April, 2022. Data collection is finished and data analysis are ongoing.  

Financing

The project is financed by the Norwegian Research Council, covering two PhD students and one 50% researcher

Collaboration

The study is a collaborative project between The University of Oslo and Akershus University Hospital.
