  This book uses sustainability to explore the interfaces between translation studies, the cultural history of knowledge, and Science and Technology studies (STS). The volume examines various material, cultural and epistemic translation practices where sustainability serves as a boundary object between natural and cultural inquiry. By turning to the intellectual traditions that influenced but were left behind by STS and actor-network theory (ANT), we aim to challenge and expand the Sociology of Translation developed in ANT. Concepts such as ‘inscription’ (Derrida), ‘actant’, ‘narrative’ (Greimas), and ‘world/worlding’ (Heidegger, Spivak) were reemployed – translated – in the canonical STS-texts. What networks of meaning were left behind in this reemployment? The book showcases a combination of cultural and knowledge historical perspectives on the construction of the Sociology of Translation and practical experiments across the registers of nature and culture is novel. There have been brilliant individual attempts to realign the Sociology of Translation with narratives and modes of enunciation, but none has related the Sociology of Translation to the networks and traditions which enabled it but to which it erased its relations and debts.

  Elimination of mother-to-child transmission of hepatitis B virus (HBV) requires infant immunoprophylaxis and antiviral prophylaxis for pregnant women with high viral loads. Since real-time polymerase chain reaction (RT-PCR), a gold standard for assessing antiviral eligibility, is neither accessible nor affordable for women living in low-income and middle-income countries (LMICs), rapid diagnostic tests (RDTs) detecting alternative HBV markers may be needed. To inform future development of the target product profile (TPP) for RDTs to identify highly viremic women, we used a discrete choice experiment (DCE) and elicited preference and trade-off of healthcare workers (HCW) in Africa between the following four attributes of fictional RDTs: price, time-to-result, diagnostic sensitivity, and specificity. Methods Through an online questionnaire survey, we asked participants to indicate their preferred test from a set of two RDTs in seven choice tasks with varying levels of the four attributes. We used mixed multinomial logit models to quantify the utility gain or loss generated by each attribute. We attempted to define minimal and optimal criteria for test attributes that can satisfy ≥ 70% and ≥ 90% of HCWs, respectively, as an alternative to RT-PCR. Results A total of 555 HCWs from 41 African countries participated. Increases in sensitivity and specificity generated significant utility and increases in cost and time-to-result generated significant disutility. The size of the coefficients for the highest attribute levels relative to the reference levels were in the following order: sensitivity (β = 3.749), cost (β = -2.550), specificity (β = 1.134), and time-to-result (β = -0.284). Doctors cared most about test sensitivity, while public health practitioners cared about cost and midwives about time-to-result. For an RDT with 95% specificity, costing 1 US$, and yielding results in 20 min, the minimally acceptable test sensitivity would be 82.5% and the optimally acceptable sensitivity would be 87.5%. Conclusions African HCWs would prefer an RDT with the following order of priority: higher sensitivity, lower cost, higher specificity, and shorter time-to-result. The development and optimization of RDTs that can meet the criteria are urgently needed to scale up the prevention of HBV mother-to-child transmission in LMICs.

  This article tests the hypothesis that all pandemics are inherently translational. We argue that translation and translation theory can be fruitfully used to understand and manage epidemics, as they help us explore concepts of infectivity and immunity in terms of cultural and biological resistance. After examining the linkage between translation and coronavirus disease from three different yet interlinked perspectives—cultural, medical, and biocultural—we make a case for a translational medical humanities framework for tackling the multifactorial crisis brought about by the SARS-CoV-2 infection. This innovative entanglement of perspectives has the merit of carving out a new space for translation research at the intersection of the sciences and the humanities, providing sustainable ways to conceptualize the production of science at times of crisis, and challenging conventional views of translation as a primarily linguistic and cultural phenomenon that traditionally does not engage with science.

  Sustainability as a concept is found across a multitude of sectors in today’s society. This ‘sustainability turn’ as we might call it, has made its entry into educational paradigms such as ‘education for sustainable development’. The healthcare sector has embraced the notion of sustainability primarily by emphasizing how climate change impacts human health. Epitomized in the new paradigm of sustainable healthcare education (SHE), or education for sustainable healthcare (ESH), the sustainability turn has arrived with full force within medical education. This article will argue that sustainable healthcare education may be analyzed as a governmental practice. We ask: by what governmental techniques does one seek to create sustainable health subjects, i.e., self-leading future doctors? On the one hand, sustainability is a call for global engagement that goes beyond the health of the singular patients within the paradigm of SHE. On the other hand, it can risk producing individual doctors and students that are responsibilized in the name of sustainability to take on ever-increasing tasks to foster human and planetary health. In this way, we argue that the SHE paradigm might risk transferring responsibility from the state to the individual to achieve ‘sustainable health’.   Rationale, Aims & Objective Patients who seek healthcare for long-lasting pain and symptoms without a detectable disease must put in extra work to be taken seriously and gain recognition as a patient. However, little is known about how patients' help-seeking is performed in clinical practice. The aim of the current study was to gain knowledge about the ways in which patients with chronic muscle pain position themselves as help-seekers during their first physiotherapy encounter. Method The material consisted of observation of 10 therapist-patient clinical interviews in primary care clinics and was analyzed using perspectives from discourse theory and the concept of positioning. Results The study highlights how the patients positioned themselves in continually shift between two discourses: that of disease (considering the patient as an object under study) and that of illness (positioning the patient as an active and participating but also troubled individual). This shifting of position was negotiated in interaction with the therapist: patients' opportunities to position themselves within the discourse of illness were limited by therapists' focus on facts and causal relationships within the discourse of disease. Conclusion Patients with chronic muscle pain seek to establish their legitimacy through the positivistic discourse of medicine and also through their compliance with the moral discourse of the patient as someone active, willing to take responsibility for their own health—and therefore worthy of treatment.

  Background Misdiagnosis is a major public health problem, causing increased morbidity and mortality. In the busy setting of an emergency department (ED) patients are diagnosed under difficult circumstances. As a consequence, the ED diagnosis at hospital admittance may often be a descriptive diagnosis, such as “decreased general condition”. Our objective was to determine in how far patients with such an unspecific ED diagnosis differ from patients with a specific ED diagnosis and whether they experience a worse outcome. Methods We conducted a prospective observational study in Bern university hospital in Switzerland for all adult non-trauma patients admitted to any internal medicine ward from August 15th 2015 to December 7th 2015. Unspecific ED diagnoses were defined through the clinical classification software for ICD-10 by two outcome assessors. As outcome parameters, we assessed in-hospital mortality and length of hospital stay. Results Six hundred eighty six consecutive patients were included. Unspecific diagnoses were identified in 100 (14.6%) of all consultations. Patients receiving an unspecific diagnosis at ED discharge were significantly more often women (56.0% vs. 43.9%, p = 0.024), presented more often with a non-specific complaint (34% vs. 21%, p = 0.004), were less often demonstrating an abnormal heart rate (5.0% vs. 12.5%, p = 0.03), and less often on antibiotics (32.0% vs. 49.0%, p = 0.002). Apart from these, no studied drug intake, laboratory or clinical data including change in diagnosis was associated significantly with an unspecific diagnosis. Unspecific diagnoses were neither associated with in-hospital mortality in multivariable analysis (OR = 1.74, 95% CI: 0.60–5.04; p = 0.305) adjusted for relevant confounders nor with length of hospital stay (GMR = 0.87, 95% CI: 0.23–3.32; p = 0.840). Conclusions Women and patients with non-specific presenting complaints and no abnormal heart rate are at risk of receiving unspecific ED diagnoses that do not allow for targeted treatment, discharge and prognosis. This study did not find an effect of such diagnoses on length of hospital stay nor in-hospital mortality.

  Pandemic modelling functions as a means of producing evidence of potential events and as an instrument of intervention that Tim Rhodes and colleagues describe as entangling science into social practices, calculations into materializations, abstracts into effects and models into society. This article seeks to show how a model society evinced through mathematical models produces a model not only for society but also for citizens, showing them how to act in a certain model manner that prevents an anticipated pandemic future. To this end, we analyse political speeches by various Norwegian ministers to elucidate how various model-based COVID-19 responses enact a ‘model citizen’. Theoretically, we combine Rhodes et al.’s arguments with Foucault’s concepts of law, discipline and security, thus showing what a model society might imply for the model citizen. Finally, we conclude that although the model society is largely informed by epidemiological models and liberal biopolitics that typically place responsibility on individual subjects, sovereign state power remains manifestly present in the speeches’ rhetoric.

  The COVID-19 pandemic has accelerated the rapid growth and financialisation of digital health, benefiting private companies in high-income countries through extractive forms of data mining. While a growing number of countries are now developing new digital strategies and policies to fill digital governance gaps, there is little public awareness and input into these processes of policy development. New models of knowledge production are required to address these inequalities and empower marginalised groups to know their digital rights in order to advocate for rights-based digital governance. The HIV response offers models for this that could be applied more widely: community-led networks of people living with and affected by HIV have translated arcane human rights law, medicine and pharmaceutical knowledge into user-friendly, actionable language, and have used this process to mobilise marginalised groups and have a meaningful say in decision-making. While the 2021 United Nations Political Declaration on HIV and AIDS calls for more community-engaged research, such approaches have yet to be applied widely in global health research. We present one model of a transnational participatory action research study into digital health for young adults in five countries and reflect on the benefits, challenges and wider implications of this approach.

  Evidence-based healthcare is the prevailing model for healthcare services. In Cochrane's seminal thinking, political context was included with the purpose of promoting healthcare equity. However, the subsequent evidence-based healthcare models marginalized political context. In this paper, we argue that current models of evidence-based healthcare fail to respond to emerging healthcare challenges. We claim that reintegration of political context is crucial to make healthcare sustainable. Global communities are anticipating ecological crises with immense repercussions for healthcare. This prospect illustrates that healthcare models failing to integrate political context also risk neglecting some of the most relevant healthcare issues of our time.

  Background Substance use disorder (SUD) is often understood as a chronic illness. Aims: This paper investigates whether SUD is treated as a chronic illness. Method To this aim, we have used World Health Organizations (WHO's) definition of chronic illness to conduct a comparative analysis of SUD and type 2 diabetes (T2D), which is another chronic illness. Results When analysing Norwegian treatment guidelines, we found that only the T2D guideline reflects the WHO's conceptualization of chronic illnesses. We argue that this discrepancy implies that SUD is understood as a moral and legal problem, while T2D is conceptualized as a somatic illness. We discuss how social, political and historical conditions of the possibility for understanding SUD are interwoven with normative presumptions about the clinician, patient, treatment guidelines and drug policies in a way that may impede the development of continuing care. Conclusion The paper concludes that the delivery of treatment services is inequitable as SUD is not treated as a chronic illness.

  The introduction of pre-exposure prophylaxis (PrEP) in 2012 in the US ushered in new technology for preventing HIV in people who are seen as at risk of contracting HIV. However, the rollout of PrEP has been filled with various debates and controversies ranging from concerns about effectiveness, adherence levels, cost-effectiveness, and moral responsibility for HIV prevention. In this context, some commentators have noted the uncanny similarities between this debate on PrEP and the debates that surrounded the launch of the oral contraceptive pill (the Pill) some 50 years ago. In this article, we provide the first to our knowledge analysis that compares how debates surrounding the launch of the Pill and debates which emerged concerning the launch of PrEP. Our analytical setting is the launch of the Pill in Norway in the 1960s and the launch of PrEP in 2016. Moreover, we wanted to focus on how both pharmaceuticals were framed in news media in Norway. We argue that such an analysis can tell us something about how the pharmaceuticalization of sex and sexuality often becomes dominated by discourses of morality and pivots around various issues of responsibility, in particular when it comes to female and gay sexualities.

  In this article, and the topical collection accompanying it, we aim to challenge socalled knowledge translation (hereinafter KT) in medicine and healthcare. The abbreviation ‘KT’ refers to a variety of scientific practices and research activities, bound together by the common goal of ‘bridging the gap’ between science in laboratories and clinical application, and, more generally, putting research-based knowledge into policy and practical care. Our objective, then, is to challenge KT by working through and with the convergence and divergences between different translational epistemologies. As KT has had a massive impact on practical healthcare, global health, and knowledge policy, as well as governance relating to sustainability, a critical examination of KT is of huge academic and societal significance. The point of departure for the contributors to this collection is the observation that KT is based upon a reductive understanding of translation and knowledge transmission. Standard models of KT take translation and knowledge transmission as a phenomenon for granted, and accordingly downplay the complexity of translation as an entangled material, textual and cultural process, which inevitably affects the ‘original scientific message’. By contrasting KT with historical, cultural, and epistemic differences from its scientific ‘prehistory’, and by analysing it with reference to broader humanistic and material views of translation, we aim to develop concepts of medical translation that can cope with contemporary epistemic and cultural differences.

  This book uses sustainability to explore the interfaces between translation studies, the cultural history of knowledge, and Science and Technology studies (STS). The volume examines various material, cultural and epistemic translation practices where sustainability serves as a boundary object between natural and cultural inquiry. By turning to the intellectual traditions that influenced but were left behind by STS and actor-network theory (ANT), we aim to challenge and expand the Sociology of Translation developed in ANT. Concepts such as ‘inscription’ (Derrida), ‘actant’, ‘narrative’ (Greimas), and ‘world/worlding’ (Heidegger, Spivak) were reemployed – translated – in the canonical STS-texts. What networks of meaning were left behind in this reemployment? The book showcases a combination of cultural and knowledge historical perspectives on the construction of the Sociology of Translation and practical experiments across the registers of nature and culture is novel. There have been brilliant individual attempts to realign the Sociology of Translation with narratives and modes of enunciation, but none has related the Sociology of Translation to the networks and traditions which enabled it but to which it erased its relations and debts.

  This book interrogates the assumption that evidence means the same thing to different constituencies and in different contexts by outlining a more nuanced and socially responsive approach to medical expertise that incorporates scientific and lay processes of making sense of the world and deciding how to act in it. In so doing, it provides a point of orientation for clinicians working at the coalface, whose experience is sometimes at odds with the type of rationality that underpins evidence-based medicine and that guides researchers conducting randomized controlled trials. The argument elaborated also has implications for policy makers in the healthcare system, who have to navigate similar pressures and contradictions between scientific and lay rationality to produce meaningful guidelines in the midst of a runaway pandemic. Debates within and beyond the medical establishment on the efficacy of measures such as mandatory face masks and lockdowns are examined in detail, as are various degrees of hesitancy towards vaccines and other pharmaceutical interventions. The authors demonstrate that it is ultimately through narratives that knowledge about medical and other phenomena is communicated to others, enters the public space, and provokes discussion and disagreements. Importantly, effective narratives can enhance the reception of that knowledge and reduce some of the sources of resistance and misunderstanding that continue to plague public communication about important medical issues such as pandemics.

  Introduction Antimicrobial resistance (AMR) has a critical global impact, mostly affecting low- and middle-income countries. A major knowledge gap exists in understanding the transmission pathway of the gut colonisation with AMR bacteria between healthy humans and their animals in addition to the presence of those AMR bacteria in the surrounding environment. A One Health (OH) approach is necessary to address this multisectoral problem. Methods and analysis This cross-sectional, mixed-method OH study design will use both quantitative and qualitative methods of data collection. Quantitative methods will be carried out to assess the prevalence and risk factors associated with multidrug resistant Gram-negative bacteria and vancomycin-resistant enterococci in humans, animals (cattle) and the environment. The focus will be on cattle rearing as an exposure risk for AMR among humans. The assessment of AMR in the population of Jimma, Ethiopia with or without exposure to cattle will reinforce the importance of OH research to identify the impending exchange of resistance profile between humans and animals as well as its ultimate dissemination in the surrounding environment. The targeted semistructured key stakeholder interviews will aid to strengthen the OH-AMR surveillance in Ethiopia by understanding the acceptability of an integrated AMR surveillance platform based on the District Health Information Software-2 and the feasibility of its context-specific establishment. Ethics and dissemination The study has been approved by the Regional Ethics Committee, Norway, and the Institutional Review Board of Jimma University, Ethiopia. The study’s data will be stored on a secure server known as Services for Sensitive Data hosted by the University of Oslo. In addition, the new European Union Global Data Protection Guidelines for data sharing, storage and protection will be followed. We will publish the results in peer-reviewed journals and present the findings at national and international conferences.

  This Collection aims to develop contemporary knowledge translation (KT) in medicine by challenging it with current cultural and humanistic theories of translation. In the process of doing this, however, we will also challenge theories of translation within the humanities by juxtaposing them with the scientific practice of KT. Different notions of “translation” have become increasingly important in the contemporary natural and human sciences. The turn to translation can be traced across a number of human sciences, such as cultural studies, anthropology and science and technology studies (STS). Translation has lately also become institutionalized in the field of medicine, leading to the development of so-called knowledge translation and ‘translational research’. These concepts refer to a set of research activities bound together by the common goal of “bridging the gap” between science in laboratories and clinical application – and more generally, putting research-based knowledge into practice. While translation in the human sciences has emerged as a key theoretical concept, and could be seen as an index of current epistemological predicaments and the almost obligatory requirement to cross-disciplinary and cultural boundaries in a “global age”, its materialization in medical discourse is of an entirely different nature. KT denotes a scientific and purportedly non-cultural practice that defines social and cultural difference as a “barrier” to the transmission of the logos of medical science. The aim of KT is to bring “pure” scientific knowledge from “bench to bedside” by testing its validity in clinical practice – while at the same time keeping the scientific knowledge intact throughout the process of translation across various social fields and sectors of the healthcare system across the globe.
